Information and advice on support and services that are available within the Solihull borough.

Adult Social Care – If you need support, click ‘Request Support' to complete one of the following assessments:

• Needs Assessment – For adults who may require care and support due to illness, disability, or frailty.
• Carers Assessment – For individuals who provide unpaid care to a family member or friend.

You can complete either assessment:

• For yourself
• On behalf of someone else, with their permission
• As a professional, supporting someone in your care

Once completed, submit the form to us so we can review your situation and offer appropriate advice and/or support.
