Adult Social Care – If you do require further support, you can click on ‘Request Support' to complete an Adult Social Care Assessment to tell us about your current situation and any support you may need. You can complete this assessment yourself, as a professional or on behalf of someone else with their permission and submit them to us.
Safeguarding – Click Report Safeguarding to complete a Safeguarding Referral if you have concerns that abuse has or is taking place. You can also contact us on 0121 704 8007 if preferred.
