Adult Social Care – If you do require further support, you can click on ‘Request Support' to complete a Carers Assessment or an Adult Social Care Assessment to tell us about your current situation and any support you may need. You can complete this assessment yourself, as a professional or on behalf of someone else with their permission and submit them to us.
Safeguarding – Click Report Safeguarding to complete a Safeguarding Referral if you have concerns that abuse has or is taking place. You can also contact us on 0121 704 8007 if preferred.
